About support coordination and case management

We offer Support Coordination Level 1, Level 2 and Level 3 services including Specialist Support Coordination. These supports ensure smooth life transitions including housing, education to formal disability supports and more. These services are delivered with a focus on being a central point of contact for our clients to ensure we make the complex simple and assist with navigating NDIS plan budgets including plan implementation, engagement of relevant services, setting up Service Agreements, regular check in on NDIS plan usage and end of plan reporting.
